INFORMATION FORM AND PROTECTION OF CUSTOMER PERSONAL DATA

The purpose of this form is to inform our Customers of the new legal provisions under the new Regulation EU 2016/679 on the protection of personal data and Law no. 2018-493 of 20 June 2018.

In application of the above provisions, in force since 25 May 2018, it is reminded that the personal data requested from our Customers is necessary to process the services and/or orders they request us to perform within the framework of the commercial commitments that have been entered into, and that the collection of this data has this sole purpose: the processing of orders and contractual services.

The purpose of this processing is therefore mainly of an accounting, commercial and logistical nature with a view to executing said order or contract. If we wish to use your data for promotional or advertising purposes, or for invitations to events or trade fairs, your prior consent shall be requested.

This data is not communicated to third parties to the contract or the order, the only recipients being our company staff members, and the various parties and service providers involved in the contract, e.g. for transport and delivery. All persons having access to this data shall be bound by an obligation of confidentiality specifically referring to respect of confidentiality of the personal data to which this person shall have access. Likewise, only certain persons in our company shall be authorized to access personal data in customer files and will have a specific password for this purpose.

This data to be provided will be mentioned as compulsory or optional, it being specified that failure to reply may impede the processing of all or part of your order or contract.

This data is kept for the legal holding period stipulated for each category of data. In accordance with the national and European regulations in force, the Customer has a permanent right of access, modification, rectification, opposition and portability with regard to the information concerning him/her. 

This right may be exercised under the conditions and according to the terms and conditions that shall be communicated to you by our Data Controller, Mrs DEREI, who can be contacted by email at the following address: rgpd@fim-medical.com.

The Data Controller shall reply within a maximum of one month and shall have the possibility of extending the deadline by a further two months depending on the complexity and number of requests, provided that the data subject is informed within one month of his or her request for right of access. If the request is incomplete (e.g. absence of an identity document), the Data Controller is entitled to request additional information: the deadline is then suspended and runs again once this information has been provided.

This form is given to the Customer at the same time as the order form and the communication of our General Sales Terms and Conditions.
